1. Do you currently have a need for an after-school care program? (Check those that apply.)

a. Full time (M, T, TH, F 3-6:30 and W 2-6:30)

b. Part time (Daily but before 6:30)

c. Weekly, but only for intermittent weeks

d. Occasionally, a few hours in a week

e. May use 1 or more of the above (a – d) depending on circumstances

f. No need at this time but would use it if the need should arise

g. Would not use an after school care program at all
